GUIDE Model Program: For individuals attributed (or pending attribution) to the GUIDE Model program. People referred to the GUIDE program should expect a call from Jade from 332-330-3891.
PIN/CCM Program: For Primary Illness Navigation or Chronic Condition Management. People referred to the PIN/CCM program should expect a call from Jade from 332-322-9226.
IBX Supplemental Benefit: For Independence Blue Cross (IBX) members and caregivers eligible for Carallel. People referred to the IBX Supplemental Benefit program should expect a call from a Carallel Care Advocate from 312-313-2340.
